I have had a life insurance policy since July of 1991 and have been paying my premium accordingly. August 10th we received to extend our policy for 10 yrs. I called on July 28th. to do so and was instructed that we would get the form to extend the policy in the next 7 to 10 business days.

My wife had been waiting for the forms so we could complete the 10 yr extension. I had to call again on August 15th as we still had not recieved the forms. August 18th the forms arrived and we unfortunately misplaced them.

I am resending my request changing my banking information for my monthly payment for my life insurance policy LT1805464.

I first requested this in January, mailing the request 01/08/2023. I phoned on 01/07/23 speaking with”Jane” who advised that all information was received and was correct. She said that 2/20/23 payment should be drawn from the new Citizens Bank account.
